✅ For renewing a FREQUENTLY LOST passport, it’s treated as a new application. You’ll need to provide a police report and an affidavit of loss, with a penalty of 350 pesos.

✅ If your passport is MUTILATED or DAMAGED, it’s considered a new application. Additional requirements include an affidavit of explanation, and you must surrender the damaged passport, along with a penalty of 350 pesos.
